How to make an appointment
Appointments are made by calling during regular office hours. We invite patients to call us prior to scheduling an appointment if additional information is needed.

Patient Instructions
Please read and follow these instructions carefully. They are important to your health and safety.

  • Please call to make an appointment. (800)321-2181
  • Do not eat, drink, chew or smoke after midnight of the night before the procedure if your procedure is on a Saturday.
  • Do not eat, drink, chew or smoke after 10:00 AM if your procedure is on a Wednesday or Friday.
  • Do not use drugs or alcohol for 48 hours prior to the procedure.
  • Bring extra underwear, 2 or 3 sanitary pads, and if you want, socks or slippers.
  • Do not wear makeup or nail polish. Women who wear contact lenses or dentures must remove them prior to the procedure.
  • Do not wear jewelry, or bring valuables to the clinic.
  • All patients must have a driver. We will need to see both the patient's and the driver's drivers licenses.
  • A minor must bring their birth certificate and be accompanied by their parent. The parent must bring a picture ID. If the parent's name differs from the minors', they must also bring a document explaining this (divorce papers, marriage certificate). Otherwise, they must acquire a judicial bypass from the court (certificate of emancipation). A minor may call the clinic for information on acquiring a judicial bypass.
  • Please do not bring children with you to the clinic. If possible, please limit the number of guests accompanying you to one or two.
  • If using insurance, bring your insurance card and picture ID (drivers license, passport or state-issued ID) with you.
  • No aspirin products prior to surgery.
